Use the Form Builder to create new Forms or update existing ones by adding fields, Blocks, and Sections and organizing how information appears to users.
What is a Form?
A Form is a configurable set of fields used to collect information about an Event or Incident.
Forms help standardize what information your team captures and can be used at different stages of a Workflow depending on what needs to be documented.
For example, a Form might collect:
Incident details
People involved
Property damage information
Investigation findings
Supporting documentation
Follow-up information
Forms can be reused across Workflows when the same information needs to be collected in more than one process.
Create a Form
Go to Settings > Forms.
Select + Add Form.
Confirm the Module.
Enter a clear Name for the Form.
Add a Description, if applicable.
Add any relevant Tags.
Select Add Form.

Organize your Form with Sections
Sections help group related fields and make longer Forms easier to understand.
Select + Add Section.
Add the fields that belong together.
Arrange your Sections and fields in the order users should complete them.
For example, a Property Damage Form might include sections for:
Incident Details
Damage Information
Costs
Assessment
Supporting Documentation
Arrange your fields
Organize fields in the order that makes the most sense for the user completing the Form.
Keep related information together and place the most important fields near the beginning.
You can also use Sections to break longer Forms into more manageable groups.
Remove a field from a form
Use Remove field when you no longer want a field to appear on a form.
Open the form and select the Form Builder tab.
Make sure Edit mode is selected.
Locate the field in the form layout on the right.
Select the × at the right end of the field row.
This removes the field from the form without deleting the field itself or its stored data.

Use Delete custom field to delete a custom field and its stored data.
Open the form and select the Form Builder tab.
Make sure Edit mode is selected.
Select Existing Fields and expand Custom in the left panel.
Locate the custom field you want to delete.
Select the trash can icon at the right end of the field row.
Follow any on-screen prompts to complete the deletion.
Preview your Form
When you're ready to review the Form:
Select Preview.
Review how the Form will appear to users.
Confirm that fields and Sections appear in the intended order.
Return to Edit if additional changes are needed.
Review Workflow usage
Open the Workflows tab in Form Properties to review where the Form is being used.
A Form may be associated with one or more Workflows depending on your organization's configuration.
Before making major changes to an existing Form, review its Workflow usage to understand where those changes may appear.
